REFERRING A STUDENT FOR SPECIAL EDUCATION SERVICES


I. For students in pre-kindergarten through grade 12 attending HISD schools, the referral source contacts the Intervention Assistance Team (IAT) chairperson for information necessary to initiate the referral.
II. For students referred for the Preschool Program for Children with Disabilities (PPCD), the parent completes the form Campus Referral Committee Report (Student Not Enrolled in School) at the home campus and signs necessary consents, releases, and notices. The school staff logs the referral and gives the information to the evaluation specialist or speech therapist. Referrals that indicate a need for an evaluation by multiple disciplines should be reviewed by the appropriate school Special Education Senior Manager and then forwarded to the Intake Center at the Child Study Department for evaluation.

III. For students presently attending a private school, the referral source should contact the private school evaluation specialist at the Child Study Department.
IV. For a visually impaired, auditory impaired, or deaf-blind child (from birth through age two), the referral source contacts the Manager for the Program for the Deaf and Hard of Hearing.

V. For a student admitted to a hospital setting and in need of special education services, the referral source is a staff member designated by the hospital or the staff from Community Services who has collected data as part of the hospital's intake process. The information is forwarded to the Community Service office and remains effective for the duration of the student's hospital stay.

VI. For a student requiring continued special education services after dismissal from the hospital setting, an HISD staff member (hospital teacher or Community Services staff member) collects the information and forwards it to the appropriate campus unless the student was receiving special education prior to his/her hospitalization.

VII. For a homebound student requiring services at home or in an alternative setting, a member of the Referral Committee completes the Referral Committee report and forwards it to Community Services. Such a student should not be withdrawn from the campus as the student’s enrollment continues at the campus of attendance during the provision of homebound services. Additionally, the "Report of Physical Examination" (Medical) is completed by the physician and forwarded to Community Services.

VIII. For a student entering HISD during the summer or being referred for the first time on a day when schools are not in session, the referral source contacts the appropriate school office. The assigned evaluation specialist collects the referral data.

IX. For a student with a disability transferring from another school district or state where the student has been in special education, the home school concurrently enrolls the student, verifies eligibility for special education in writing or by telephone, explains the pamphlet, Notice of Procedural Safeguards (Rights of Parents of Students with Disabilities) to the parent, and obtains a signed statement indicating receipt. The parent/guardian also receives the Notice of Full and Individual Evaluation and Test Description, signs the Consent for Full and Individual Evaluation, and completes the Sociological Data Form.
